Job Deficiency Checklist


This checklist is simply a guide for identifying possible problems. The presence of some of these conditions does not necessarily prove that job problems exist. Use the checklist as a worksheet, and discuss concerns about employee performance with Duke EAP staff, as appropriate.

A. Quality and Quantity of Work

The employee has:
  • Clearly refused to do assigned tasks.
  • Repeatedly made errors in spite of increased guidance.
  • Shown a significant increase in errors.
  • Made unreliable or false statements.
  • Substantially reduced the quantity of work.
  • Manifested inconsistent, "up and down" quantity and/or quality of work.
  • Engaged in behavior that disrupts workflow.
  • Often procrastinated on significant decisions or tasks.
  • Often required more than usual supervision.
  • Often provided unsupported explanations for poor work performance.
  • Demonstrated a noticeable change in written or verbal communication.
  • Other (please specify)
B. Interpersonal Work Relationships

The employee has:
  • Shown significant change in relations with co-workers, supervisors, and/or others.
  • Engaged in frequent or intense arguments.
  • Exhibited verbal abusiveness.
  • Exhibited physical abusiveness.
  • Often appeared withdrawn or less involved with people.
  • Avoided contact with supervisor.
  • Frequently expressed frustration or discontent.
  • Shown significant change in frequency or nature of complaints.
  • Been the subject of complaints by co-workers or subordinates.
  • Made cynical, "distrustful of human nature," comments.
  • Demonstrated an unusual sensitivity to advice or critique of work.
  • Responded unpredictably to supervision. (please specify ___________)
  • Exhibited a passive-aggressive attitude or behavior, (i.e., "doing things behind your back").
C. General Job Performance

The employee has:
  • Had excessive unauthorized absences - # in last 12 months. _______.
  • Had excessive authorized absences - # in last 12 months. _______.
  • Had excessive use of sick leave - # in last 12 months. _______.
  • Had frequent Monday/Friday absences or other pattern.
  • Had frequent unexplained disappearances.
  • Had excessive "extension" of breaks or lunch.
  • Frequently left work early - # of days per week or month. _______.
  • Been the subject of increased concern about, or has been involved in actual incidents or safety offenses _____________.
  • Experienced or caused job accidents.
  • Had a major change in duties or responsibilities.
  • Interfered with or ignored established procedures.
  • Demonstrated an inability to follow through on job performance recommendations.
D. Personal Matters

The employee has:
  • Exhibited changes in or unusual appearance (i.e., dress, hygiene).
  • Exhibited changes in or unusual speech (e.g., incoherence, stuttering, volume, or tone).
  • Exhibited changes in or unusual physical mannerisms (e.g., gestures, posture).
  • Exhibited changes in or unusual level of activity (i.e., much reduced or increased).
  • Engaged in unusual topics of conversation.
  • Engaged in detailed discussions about death, suicide, or harming someone.
  • Become increasingly irritable or tearful.
  • Exhibited persistently boisterous or rambunctious behavior.
  • Manifested unpredictable or out of context displays of emotion.
  • Expressed unusual fears.
  • Demonstrated a lack of appropriate caution.
  • Engaged in detailed discussion/s about obtaining or using drugs and/or alcohol.
  • Experienced personal relationship problems (e.g., with spouse, girl/boyfriend, children, in-laws) that negatively impact work performance (e.g., spending time away from work station, or increasing the number of personal phone calls).
  • Made unfounded accusations (i.e., has demonstrated unfounded feelings of persecution and/or paranoia).
  • Engaged in secretive or furtive behaviors.
  • Exhibited memory problems (e.g., difficulty recalling instructions, data, past behaviors).
  • Frequently been sick with colds, flu, or other illnesses.
  • Come to work with alcohol on breath.
  • Expressed excessive fatigue.
  • Made unrealistic self-appraisals or grandiose statements.
  • Engaged in temper tantrums or angry outbursts.
  • Exhibited a demanding, rigid, inflexible attitude.
  • Experienced a major change in physical health.
  • Expressed unfounded concerns about sexual behavior or sexual harassment.
