Network of Providers


Duke EAP has provided services to companies both locally and nationally. Our external provider network now includes more than 4,700 providers which we have personally credentialed and periodically maintain their eligibility. As we work with specific corporate clients, we continue to customize our network to provide customer-focused access to providers who meet our standards.

Duke EAP identifies providers who have established positive community reputations and invites them to apply. We make every effort to maintain a culturally diverse network with a wide range of clinical expertise. Selection criteria for EAP professionals centers around appropriate professional certification and/or licensure, experience and excellence in clinical service delivery, and commitment to improving mental health in the workplace. Our professionals typically have extensive experience in brief counseling and/or substance abuse assessment. Solicited, as well as unsolicited, applications are accepted and then extensively reviewed to assure that clinicians meet Duke University Medical Center's credentialing requirements. All providers must complete and/or provide our office:
  1. A Duke University Medical Center Employee Assistance Professional Application. This application includes a section in which the clinician must answer seventeen specific questions concerning his/her clinical and professional history.

  2. A photocopy of his/her state license, resume, and liability insurance (malpractice coverage must have a minimum of $1,000,000 Limits of Liability and $3,000,000 Aggregate).

  3. A Graduation Verification Form, State Licensure Verification Form (identifies any restrictions placed on the provider's license) and Insurance Verification Form (identifies any claims against the provider's malpractice insurance).

  4. One professional reference who can provide information on the applicant's training and clinical practice. The reference is provided a standard Duke University Medical Center Reference Request Form for completion.
Completed applications are thoroughly reviewed by the Duke EAP Credentialing Committee. This internal committee consists of two licensed clinical social workers, one licensed clinical psychologist, and the Provider Relations Coordinator. Any questionable information, license restrictions, and/or claims against a clinician's liability insurance are further researched and Duke's Risk Management Office consulted if necessary. If approved by the Committee, a final national license check is completed through the National Practitioner's Database. Provided the record is clear, the clinician is accepted into the network. Once accepted, a formal agreement is sent to the provider for his/her review and signature.
